Aims

To provide support to adult carers on a district wide basis.

Objectives

To encourage and enable GP practices & Primary Care Trust to identify and support their patients who have a caring role.
To improve accessibility to information, advice, counselling, support and assessment for all carers.
To continue to forge new links and improve communication channels between statutory and voluntary organisations.
To provide information on existing, and new services to carers, in particular for carers who may have special requirements, for example, their youth, ethnicity or lifestyle.
To work in partnership with Adult Social Care, Health & other relevant organisations in order to meet carers unmet needs.
To ensure that Adult Social Care, and GP’s have an awareness, and better knowledge of the needs of carers; that carers' expertise and competence is acknowledged, and a willingness is demonstrated to regard them as partners in the caring process.
To consult with carers on the effectiveness of the service.
To ensure close partnership working with Primary Care Groups and Primary Care Trust. Carers Support Worker’s are jointly funded by the Health Authority, Department of Adult Social Care and managed by the Carers Support Manager.
