This service for adult, concerned families and friends from 18 upwards. There is a seperate service for Young Carers.
If you are looking after a relative, friend or neighbour and you feel in need of support, information and advice or just a listening ear, this service is for YOU.
Carers can ring the Carers Support Worker direct. GP’s, Healthcare Professionals and Social Services Workers can ring direct, complete a referral form or write a letter to the Carers Support Worker.
Yes! But the Carers Support Worker is there to support YOU as a Carer. You may often feel that your needs are overlooked or that no-one has time to listen to you. Our role is to ensure that you get the help, support and information and advice that you need and deserve.
Yes, absolutely. We may ask your permission to give feedback to your GP or other Healthcare Professionals involved in your care. This information will help your GP to better understand your situation. The Government are also keen that GP practices keep a record of those who have a caring role so that you can be offered a better and more sensitive service. We will always ask your permission to do this.
The Carers Support Worker will contact you and arrange a time to meet.
If you would like to know more about the services they can provide please go to the bottom of this page for a list of contacts.
